Extra Charge for International Orders

Is it possible to add an extra charge to any orders outside of the United States? It would be best if we could charge different fees based on the country selected, but at a minimum being able to charge an extra base fee to any country outside of the United States would be acceptable.

Would you want to treat that as a handling charge on the order or just simple add X dollars to the shipping costs based on each country? Either way, it will have to be customized in source code.
